Submit an incident from the store floor

Summary: As a store associate, you will use the simplified store-floor form to file an incident, and you can review the incidents recorded at your own stores.

Who this is for

This article is for the Store Associate role: store-floor staff with a signed-in account confined to intake and a read-only view. When you sign in, you land directly on the incident form. Your navigation is a small strip of links (Submit Incident, Report Accident, and My Store Incidents) and every one of those pages renders in the same stripped-down chrome. You never see the full app.

This is not an anonymous form. You sign in like any other user; there is no unauthenticated way to file. If you are not signed in, you are sent to the sign-in page first.

File an incident

  1. Sign in. You arrive on the incident form.
  2. Confirm the store. The form asks your browser for your location and pre-fills the nearest store. If you have assigned stores, both the pre-fill and the store picker offer only your own stores; if nobody has assigned you a store, you see every store and can file anywhere, so an assignment gap never blocks a report.
  3. Fill in the incident details. You can type, dictate by voice, or scan product barcodes: all three work here.
  4. Submit. A confirmation appears in place.

Read your own stores' incidents

Select My Store Incidents to see the incidents recorded at your assigned stores. This list and its detail pages are read-only for your role: there is no New Event action, no edit, and no delete. If nobody has assigned you a store, this list is empty, but you can still file.

Good to know

  • You cannot edit an incident once filed: the list and detail views are read-only for you. If a correction is needed, tell your asset-protection contact, who can edit it.
  • You can also file accident reports. The Report Accident link opens the accident intake form. Accidents work differently: you file one and cannot see it again afterward. See File an accident report.
  • First-time timezone prompt. The first time you reach the form, you may be asked to confirm your timezone. This appears once and cannot be dismissed without answering, because it keeps every timestamp you see correct.
