Troubleshooting: accident reports
I filed an accident report but can't find it
If you are a store associate, this is by design: your role files an accident and cannot see it again (no list, no detail, not even the report you just filed). The confirmation's quotable reference is your record that it was filed; Risk and HR are emailed on every filing. Ask your Risk or HR contact if you need the report itself.
If you are a Regional or District Asset Protection Manager, check that the report's store is inside your territory: a report at an out-of-territory store, or one tied to no store, is not visible to you.
I can't correct or delete a report I filed
Correcting or deleting any part of a filed report requires the Claim access band. Regional and District Asset Protection Managers and Store Coordinators can create a report but hold only the Scene band, so they cannot change it once filed. Ask someone with the Claim band (an Administrator, Supervisor, or Manager) to make the correction.
A report can't be saved half-finished
There is no draft save. The intake form has a single Create Accident Report action that stays disabled until every required field is answered, and leaving mid-report loses what you typed. Complete the report in one sitting, or gather the required scene facts before you start. (This is a known limitation the product team is tracking.)
The claim tab says "Awaiting configuration"
The report cannot resolve a value from your organization's setup:
- "Awaiting organisation configuration": the employer profile is missing or has no FEIN. An Administrator fixes it on the employer profile page.
- "Awaiting store configuration": the store has no carrier linked, or no state or address. An Administrator links a carrier from the store's edit page, or corrects the store record.
- "Awaiting a pick among N linked carriers": the store has several carriers and no pick has been made. Anyone who can edit the report picks one.

The TPA package button is disabled
- On a workers'-comp report, the button is gated on completeness: the Exports tab lists the outstanding fields, grouped by tab. Fill them in (fields that depend on the passage of time, such as a return-to-work date, are not required).
- On a general-liability report, there is no TPA package at all: the button is disabled for that reason, with no field list.
I can only export the Store copy
You hold only the Scene band. The Store copy is the one document a Scene-band reader is entitled to; the other four each need at least the Claim band. See Access bands.
The report calls itself an "Incident"
Some statutory forms carry "Incident" in their own titles and field names ("Incident Date & Time", "Incident Only"). That is the external form's wording: an accident report is never a crime incident, and the two are never linked or counted together.
