Submit an accident from the store floor
Summary: As a store associate, you will file an accident report through a stripped-down form, and know that you cannot see it again afterward.
Who this is for
This article is for the Store Associate role: store-floor staff, including the shift supervisors and assistant managers who file accidents at the scene. You reach accident filing from the Report Accident link in your navigation strip (alongside Submit Incident and My Store Incidents). Everything renders in the same stripped-down chrome as your other forms.
File an accident
- Select Report Accident.
- Confirm the store. If you have assigned stores, the picker and the "nearest store" suggestion offer only your own stores; if nobody has assigned you a store, you see every store and can file anywhere.
- Answer the track question and complete the required fields, including the mandatory photograph. The same completeness gate applies as for any filer: see File an accident report.
- Submit. A confirmation appears showing a reference you can quote.
Submit and forget
Your role files an accident and then cannot see it again: you have no accident list, no detail view, and no access even to the report you just filed. The confirmation shows a quotable reference, never a link back.
This is by design, not a fault: the accident record is protected by the access-band layer (your role holds no band), so filing is write-only for you. Risk and HR are emailed on every filing, so "submit and forget" never means "submit into silence": someone with the right access picks it up.
What you can and cannot do
- Can: file incidents, file accident reports, and read your own stores' incident list and details.
- Cannot: view, edit, or export any accident report, including your own, and reach any other part of the platform.
